How it was built

1. Extract. Each subject starts from a widely used Anki deck. Every card becomes a fact keyed by a hash of its own text, so a rebuilt deck never orphans a question.
2. Author. A fleet of AI agents writes one board-style question per fact against a shared spec. The deck is the source of truth, with one exception: if a card is wrong in the real world, the agent writes the medically correct question and files a discrepancy.
3. Review. A second, independent pass reads each draft beside its source card and rejects ambiguous distractors, nested thresholds, and stems that give the answer away. A third pass by a different model family can only flag, never edit, and every flag gets a human ruling.
4. Ship. A deploy runs only after dataset assertions pass. Explanations keep their source card and references so any question can be traced.

The finding worth knowing about. Roughly 100 factual errors surfaced in the commercial source decks during authoring, some with real guideline weight behind them, and were corrected here. If you drill the original decks, the cards are not always right.
